Despite extensive research efforts to unravel the mysteries of SARS-CoV-2, the virus continues to defy our attempts at comprehension. Despite our attempts to fit it into the established patterns of respiratory viruses, it refuses to conform. Predictions of a strictly seasonal pattern have proven incorrect, as the virus persists throughout the year and exhibits multiple waves, often in late summer. Expectations of a single annual vaccine in autumn have also been disrupted. Additionally, some variants initially feared to be severe have turned out mild, while others have presented significant challenges. The Covid-Boosters remain enigmatic, confounding our attempts to understand and control it.

Surprising study results

A new puzzle has emerged in the ongoing battle against COVID-19. Prior to the emergence of the Omicron variant, it was widely believed that a combination of vaccination and previous infection, known as hybrid immunity, offered the most robust protection against future infections. However, our research group, which has been studying vaccinated older adults in long-term care and retirement homes throughout the pandemic, has made a surprising discovery. We found that individuals who had previously fought off the BA.1-2 variant of Omicron in early 2022 actually had a significantly higher risk, 30 times higher, of contracting the BA.5 variant later in the same year. This unexpected finding challenges our previous assumptions and raises new questions about the dynamics of the virus.

We must remain vigilant

Our research has been possible due to our study participants residing in long-term care and retirement homes, a group that undergoes frequent testing, has high vaccination rates, and is closely monitored. The reduction in frequent PCR testing and documentation of infections in the broader population made it challenging to gather comprehensive data on COVID-19 infections and reinfections. Thankfully, these seniors have provided valuable insights that would have otherwise been overlooked.

From our observations, it’s clear that the virus has evolved in a way that makes previous infection no guarantee of immunity from future ones. While there is still much we don’t fully understand about COVID-19, such as its long-term health effects and the mechanics of its ongoing mutations, one thing is certain: we cannot afford to lower our guard. 

Our findings highlight that vaccines can help mitigate the severe consequences of subsequent COVID-19 infections, but the virus continues to adapt, finding new ways to evade our immune defences. This underscores the importance of ongoing vigilance and research in our fight against the virus.

Ensuring our safety and that of our communities

Are masks and boosters still necessary? Yes, despite the weariness they may cause, they remain essential, particularly for our most vulnerable populations: older adults, individuals with chronic illnesses or compromised immune systems, and those who care for them. It’s important to recognize that the protective effect of multiple COVID-19 vaccines doesn’t accumulate like savings in a bank account. Instead, the timeliness of our booster shots determines our level of protection.

While not perfect, timely boosters serve as our primary defence. We should view them less like childhood vaccines, which provide long-lasting immunity, and more like annual flu shots that require regular updates to align with the circulating strains. These boosters may reduce the risk of symptomatic infection for a few months but, critically, help prevent severe illness and hospitalization.

We must stay vigilant and ensure our vaccinations are up to date to safeguard against COVID-19 infections and their potential long-term consequences, even mortality.
